City Clerk’s Office begins annual city census

The Office of the City Clerk has mailed the annual city census to all residential households as required by Massachusetts General Law Chapter 51 Section 4. Your response ensures that your voting status is active and helps the city provide better municipal services. Even if you’re not a registered voter, responding will assure the Office of the City Clerk can provide proof of residency. 

Residents should complete the form accurately and be sure to list each person living at the address as of Jan. 1, 2025. The form will be prefilled with data from last year. Any changes should be marked on the form. The form should then be signed, dated and returned to the Office of the City Clerk in the envelope that comes enclosed with the form. 

Any changes to a voter’s registration will require the voter’s signature.

If there are no changes to the form, residents can respond online using the unique ID shown under the barcode at the top right corner of the census form (see the example below). Click here to respond online.

Residents who have not received a census form can click here to download and print a blank form. The form can be returned by mail or dropped off in the secure city mailbox in the rear of 610 Main St. The postal address to mail the form is
